is everyone constantly misunderstanding the world and each other, or am i just projecting when i say that nobody can see anyone for who truly they are, not even themselves?

neither, or rather a mix of both. people are so unique that it's kind of hard to understand someone when they're different. sometimes, even a small difference is hard for some people, so we get these wild misunderstandings for really trivial things.

adding another layer, is everyone just projecting constantly as a substitute for being able to truly understand the world and the people in it, or am i just projecting?

it's whatever world view you want to project. as long as you believe it, it's real. for you at least. which i'd consider real.
